    Summary

    Occupational therapy is a health care profession that helps individuals with difficulties achieve their maximum level of independence and potential in all areas if daily living, including work, play, self-care, and school. The service works with other key people involved with children and young people’s lives such as families, other health professionals, education and social care to ensure the best support is available.

    The service helps children and young people who have significant difficulties carrying out their everyday activities, are aged 0 - 18 years and live in Pembrokeshire, Carmarthenshire or Ceredigion. 

    How to access the service:


    1. Referral by GP / Health Professional

    You can be referred to the service by health professionals that know the child.

    2. Self Referral via Form

    You can complete a children’s occupational therapy request for help form: Paediatric occupational therapy - electronic referral form 

    Usually the first assessment is carried out over the telephone so please ensure all the numbers are correct and that you have stated any communication or language needs. Please also include an email address.  Requests may be returned if more information is needed.
